We’re growing, and we’re looking for the right nurse to grow with us!
As our hospice census continues to grow in the Salt Lake City area, we are looking for a compassionate and dependable Registered Nurse Case Manager to join our team.
We have a wonderful team of professionals who genuinely care about their patients, their families, and each other. We believe hospice care is about more than managing symptoms—it’s about helping people live with comfort, dignity, and support during one of the most important times in their lives.
We are looking for an RN who shares that philosophy and wants to make a meaningful difference in the lives of patients and families in our community.
What You’ll Do:
As a Hospice RN Case Manager, you will coordinate and provide individualized nursing care for a caseload of hospice patients in their homes and residential care settings throughout the Salt Lake City area.
- Perform comprehensive nursing assessments and ongoing evaluations of hospice patients.
- Develop, implement, and update individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.
- Provide skilled nursing care with a strong focus on comfort and symptom management.
- Recognize changes in patient condition and respond appropriately and promptly.
- Educate and support patients and families regarding medications, symptom management, disease progression, safety, and what to expect throughout the hospice journey.
- Coordinate care with LPNs, CNAs, social workers, chaplains, physicians, facilities, pharmacies, DME providers, and other members of the hospice team.
- Communicate changes in condition and patient needs to the appropriate members of the interdisciplinary team.
- Participate in interdisciplinary group (IDG) meetings and collaborate in developing and updating the plan of care.
- Complete accurate and timely clinical documentation.
- Help ensure patients and families have the medications, equipment, supplies, education, and support they need.
- Provide guidance and supervision to LPNs and hospice aides as appropriate.
- Participate in the on-call rotation as scheduled.
- Maintain compliance with hospice regulations, agency policies, and professional nursing standards.
What We’re Looking For:
- Current Registered Nurse license in the State of Utah.
- Graduate of an accredited nursing program.
- Nursing experience preferred; hospice, home health, palliative care, oncology, geriatrics, or other relevant experience is a plus.
- Strong clinical assessment and critical-thinking skills.
- Compassionate and comfortable caring for patients and supporting families at end of life.
- Excellent communication and interpersonal skills.
- Strong organization, time-management, and prioritization skills.
- Ability to manage a caseload while recognizing and responding to changing patient needs.
- Ability to work independently in the field while also being an active member of a collaborative team.
- Dependable, flexible, and committed to following through for patients and families.
- Current driver’s license, reliable transportation, and automobile insurance.
The Kind of Nurse We Hope to Find
Clinical skills are important, but who you are with patients and families matters just as much to us.
We are looking for someone who understands that sometimes the most important part of a hospice visit is listening. Someone who will advocate for their patients, communicate with their team, support overwhelmed families, and take the extra moment to make sure a patient is comfortable before leaving.
Our team truly cares about the people we serve, and we want to add a nurse who feels the same way.
Why Join Our Team?
We are hiring because we are growing. As we serve more patients and families throughout the Salt Lake City area, we want to grow our clinical team thoughtfully so our nurses have the support they need to continue providing excellent, compassionate care.
You won’t be doing this work alone. Hospice is a team effort, and our nurses work alongside an interdisciplinary group of professionals who share the same goal: giving patients the best possible quality of life, comfort, and dignity for the time they have.
If you are an RN looking for meaningful work and want to be part of a team that truly cares about its patients, families, and one another, we would love to meet you.
