We are looking for a compassionate, dependable Licensed Practical Nurse (LPN) to join our hospice team.
Our LPNs provide hands-on nursing care to hospice patients in their homes and residential care settings while working closely with the RN Case Manager and the rest of our interdisciplinary team.
Hospice nursing is about more than completing a visit. It is about helping patients remain comfortable, supporting families during an important time in their lives, recognizing changes that need attention, and making sure patients and caregivers know they are supported.
Mileage reimbursement is provided.
What You’ll Do:
- Provide skilled nursing visits to hospice patients as assigned and according to the established plan of care.
- Perform nursing treatments and procedures within the LPN scope of practice and under RN supervision.
- Observe patients for changes in physical or emotional condition and promptly communicate concerns to the RN and appropriate members of the care team.
- Provide medication, symptom-management, safety, and care education to patients and caregivers.
- Teach and support families with appropriate patient care and self-care techniques.
- Complete accurate and timely clinical documentation following each visit.
- Communicate effectively with RN Case Managers, physicians, facilities, caregivers, and other members of the interdisciplinary team.
- Coordinate with the care team to help ensure patient needs are addressed promptly.
- Participate in interdisciplinary team meetings and care coordination as needed.
- Follow established hospice policies, procedures, and the individualized plan of care.
- Help ensure patients and families receive compassionate, respectful, and responsive care.
What We’re Looking For:
- Current LPN license in the state of practice.
- Graduate of an approved practical nursing program.
- Nursing experience preferred; hospice, home health, long-term care, or geriatric experience is a plus.
- Compassionate and comfortable caring for patients and supporting families at the end of life.
- Strong observation, communication, and documentation skills.
- Ability to work independently while recognizing when a change in condition needs to be communicated or escalated to the RN.
- Dependable, organized, and able to manage visits and documentation efficiently.
- Strong time-management and prioritization skills.
- Comfortable traveling to patients in their homes and residential care facilities.
- Willingness to communicate and collaborate as part of an interdisciplinary hospice team.
- Reliable transportation and ability to travel throughout the service area.
About the Position
This is a per-visit position offering flexibility while allowing you to do meaningful, patient-centered nursing work. Visits are provided in patients’ homes and care facilities throughout our service area, and mileage is reimbursed.
