We are looking for a compassionate, dependable, and organized Registered Nurse (RN) to join our Home Health team.

Our nurses have the opportunity to provide individualized care where patients are most comfortable; their own homes. Home health nursing allows you to build relationships with patients and families while using your clinical judgment, assessment skills, and nursing experience to help patients recover, remain safe, and become as independent as possible.

What You’ll Do:
As a Home Health RN, you will provide skilled nursing care and coordinate services for patients receiving care in their homes.
  • Perform initial and ongoing comprehensive nursing assessments.
  • Develop, implement, evaluate, and update individualized plans of care in collaboration with physicians and other members of the care team.
  • Provide skilled nursing services according to physician orders and the established plan of care.
  • Monitor patients for changes in condition and communicate concerns promptly to physicians and other appropriate members of the care team.
  • Provide patient and caregiver education regarding medications, disease management, treatments, safety, and self-care.
  • Perform skilled nursing procedures and treatments within the RN scope of practice.
  • Complete OASIS assessments, reassessments, resumptions of care, transfers, discharges, and other required documentation accurately and within required timeframes.
  • Coordinate services with Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, Home Health Aides, physicians, and other healthcare providers.
  • Obtain and process physician orders and communicate changes in the patient’s needs or plan of care.
  • Help patients and caregivers understand their care plan and work toward their individualized goals.
  • Complete accurate and timely clinical documentation for each visit.
  • Supervise LPNs and Home Health Aides as appropriate.
  • Participate in care conferences, quality improvement activities, and team communication.
  • Participate in the on-call rotation as scheduled.
  • Follow agency policies, professional nursing standards, and applicable home health requirements.

What We’re Looking For:
  • Current Registered Nurse license in the State of Utah.
  • Graduate of an approved professional nursing program.
  • Nursing experience preferred; home health experience is a plus.
  • Strong nursing assessment and clinical judgment skills.
  • Ability to work independently and make appropriate clinical decisions in the home setting.
  • Strong organization, time-management, and prioritization skills.
  • Excellent communication and patient-education skills.
  • Comfortable coordinating care with physicians, therapists, caregivers, and other healthcare professionals.
  • Ability to complete accurate and timely clinical documentation.
  • Experience with OASIS is preferred, or a willingness to learn.
  • Dependable, flexible, and committed to providing excellent patient care.
  • Current driver’s license, reliable transportation, and automobile insurance.
