We’re growing, and we’re excited to add a new Clinical Manager to support our team!

As our branch continues to grow, we are creating a full-time Clinical Manager position to provide additional leadership, clinical guidance, and day-to-day support to our nursing team.

We have a dedicated group of clinicians who genuinely care about their patients, their families, and each other. We are looking for an experienced Registered Nurse leader who can help our team continue to grow while maintaining the high standard of care our patients deserve.

This is a full-time salaried position with a full benefits package and paid time off (PTO).

What You’ll Do:
As Clinical Manager, you will work closely with the Director of Nursing and other members of leadership to oversee and support clinical operations for our growing Home Health and Hospice team.
  • Provide day-to-day clinical leadership, guidance, and support to RN Case Managers, LPNs, aides, and other clinical staff.
  • Serve as a clinical resource when staff have questions, concerns, or changes in patient condition that require additional guidance.
  • Help clinicians establish priorities, problem-solve challenging situations, and ensure patient needs are addressed appropriately.
  • Review referrals and assist with determining appropriate services, staffing, and assignment of clinicians.
  • Support coordination of care across the clinical team.
  • Monitor clinical documentation for accuracy, completeness, timeliness, and regulatory compliance.
  • Assist with clinical chart review and quality assurance activities.
  • Review OASIS documentation for accuracy and provide education and feedback when needed.
  • Help ensure compliance with Home Health and Hospice regulations, agency policies, and professional standards of practice.
  • Lead and participate in clinical meetings, case conferences, and interdisciplinary discussions.
  • Support staff education, competency development, and ongoing clinical training.
  • Provide coaching and constructive feedback to help team members grow professionally.
  • Assist with orientation and training of new clinical employees.
  • Participate in interviewing and selecting new clinical team members.
  • Monitor staffing needs, productivity, and clinical workloads and communicate needs to leadership.
  • Assist with employee evaluations and performance management as appropriate.
  • Participate in quality improvement and QAPI initiatives.
  • Help coordinate appropriate RN coverage and provide clinical/on-call backup as needed.
  • Collaborate with physicians, facilities, community providers, patients, and families to promote continuity and quality of care.
  • Partner with the Director of Nursing and leadership team to identify opportunities to improve clinical operations and patient care.

What We’re Looking For:
  • Current Registered Nurse license in the State of Utah.
  • Graduate of an accredited professional nursing program; BSN preferred.
  • Minimum of two years of nursing experience or equivalent relevant clinical experience.
  • Leadership or supervisory experience preferred.
  • Home Health and/or Hospice experience strongly preferred.
  • Knowledge of Home Health and Hospice clinical practices, documentation, and regulatory requirements preferred.
  • Familiarity with OASIS and Home Health documentation requirements is highly desirable.
  • Strong clinical judgment and problem-solving skills.
  • Excellent organization, prioritization, and time-management skills.
  • Strong written and verbal communication skills.
  • Ability to provide clear direction while remaining approachable and supportive.
  • Comfortable providing coaching, education, and constructive feedback.
  • Ability to balance accountability with encouragement and support.
  • Current CPR certification.
  • Current driver’s license, reliable transportation, and automobile insurance.
